If you wish to make a change to an approved research protocol, please be aware that all substantive revisions (i.e., modifications, addenda, amendments) to a project must be reviewed and approved by the IRB prior to initiation.

Principal investigators should submit a Revision Request form to the IRB and provide updated versions of any study materials (e.g., informed consent documents, data collection instruments, etc.) affected by the change. Project revisions can often be reviewed and approved by one IRB member; however, a project that was initially reviewed by a full board must be reviewed again by a full board for any revisions. Approval must be granted before any changes can be initiated.
